Aneurysm Surgery


A Stroke is the ultimate "Brain Attack." It can strike the young, healthy, productive head of a household during a soft, shared moment with a spouse -- anytime. Nothing is more frightening. One might hear: "I'm having the worst headache of my life!" Or, "Oh, my neck is so stiff!" Then the victim might vomit, perhaps have a convulsion which could leave them comatose and all bystanders truly frightened.

In the emergency room even the doctors look worried. The CAT scan shows "blood around the brain," something doctors call a "subarachnoid hemorrhage." There could possibly be other problems, such as a blot clot in the brain itself ("intracerebral hemorrhage) and/or something called "hydrocephalus" (water on the brain, building up after blood cells have gradually clogged, the brain's drainage system).

There is lots to be done, and in a very short time frame. Backtracking a bit, it is important to get an instant education on the "Brain Attack."






Some basic questions:
  • What is a brain aneurysm?
  • How serious is the problem?
  • How did this problem come about in the first place?
  • Why this specific person?
  • What are the problems at hand?
  • Who can do what? Etc. Etc. Etc.

The Aneurysm

The brain is interesting for many reasons. Of prime importance, the brain uses, pound for pound, more energy than any other part of the body. It must be guaranteed just the right amount of blood for every instant of its life in order to survive and function well. Of all of the blood that the heart pumps, the brain needs the most. It weighs about 5% of the body's total weight, yet it demands and monopolizes about 20% of the volume the heart puts out. The blood vessels at the base of the brain (these are the largest, and "smartest" of all vessels) have the capability to control -- even commandeer -- blood to supply the brain as conditions demand. When we lie down, these vessels constrict automatically to prevent the brain from being flooded with an over supply. And, when we jump up, these vessels have the James Bond-like buttons built in, to make sure that the heart not only pumps harder, but also that the other blood vessels open up enough to handle the increased volume required by the brain. Thus, these blood vessels have three things that are needed for survival:

  1. Elasticity that allows them to keep their shape (like socks)
  2. "Smooth muscle," that can be relaxed or tightened as conditions require
  3. An electronic feedback system (i.e. sensors and nerves) that makes everything work.

People who grow aneurysms have an inborn problem with the elasticity part of these blood vessels. The elastic layer is not formed properly (being randomly, rather than regularly organized). This weaker part of the blood vessel begins to bulge and balloon over the years. It is this bulge which is actually called the "aneurysm." It develops in places where the pressure is greatest -- usually where the vessels divide into multiple routes. It enlarges and enlarges, and enlarges further still. Eventually, its chance of "popping" like an over inflated balloon becomes real. When the local blood pressure exceeds the strength of that vessel's weak point, the danger of rupture is at hand.

In these people, over the years of their lifetimes, and at the points along these vessels where the defect in the elastic layer exists, a gradual bulge and finally a balloon develops in places where the pressure is greatest (usually where the vessels divide). Over time, the result is a little (and ever-enlarging) balloon called an "aneurysm." And, as for all balloons, the chance of "popping" becomes real when the local blood pressure exceeds the strength of the balloon's wall at its weakest point.

When they do pop, a surprisingly small amount of blood escapes, due to the efficiency of the blood's clotting system. It's difficult to imagine, but only 5 to 10 cc's of blood could be responsible for causing such disastrous consequences! In those people whose aneurysms bleed much more, death occurs even before they get arrive at the hospital.

"Brain Attack"

1/3rd of people with ruptured aneuryms die before they get to the hospital.

1/3rd die after they get to the hospital.

1/3rd survive after they get to the hospital. Of these, 40% end up with neurological problems that make life difficult.

Let's look at the problems that occur after arrival at the hospital. Then, the all important "how-to-prevent and/or fix" them as they arise. First...

Truth and Consequences Solution

Hydrocephalus? An insult added to the injury Place a "shunt."

Vasospasm? The 1/3 who die after coming to the "Triple H" therapy

Preventing Second Hemorrhage

In the emergency room, the doctors will try everything to prevent the small clot at the rupture point in the aneurysm from dissolving or becoming dislodged altogether. They will concentrate on three things:

  • * Keeping the blood pressure spikes form occurring again. (Give blood pressure medications, prevent anxiety attacks, keep the environment calm).
  • * Giving drugs to prevent the clot from dissolving ("Amicar" can only be used for the short-term, because it will cause blood clots in the legs and lungs to become a problem.)
  • * Avoiding convulsions (drugs such as dilantin, phenobarbitol).

Once the patient is stable, and the full support team is assembled, the definitive procedure to prevent re-bleeding can be undertaken: The Operation.--

The operation has as its aim the "clippings" of the aneurysm at the base of its neck (just like tying off a balloon), so that it is eliminated from the blood circulation of the brain.

The aneurysm can be seen by a number of techniques, the most reliable and accurate of which is an angiogram. A catheter is passed into the vessels at the base of the brain and a radio-opaque dye (called contrast) is injected while X-Rays are taken. This study sets up the road map of the blood supply to the brain, and brings the anatomy of the aneurysm to light. Other, less invasive ways to do angiography include MRI scanning (MRAngiography) and the Spiral CT Scan using intravenous rather than intraarterial contrast. These studies give a 3-D character to the images, making them particularly helpful to the surgeon.

Aneurysms can also be cured by other techniques in selected cases. A revolution (some call it a rapidly expanding evolution) is occuring in the technology of "interventional radiology," allowing doctos to fill aneurysms with metal coils, glue, balloons, and even "stents."(JR: What is a stent?) There may well be a future in which surgery will become the other option, if such rapid progress brings the hoped-for results.

The innumerable improvements and in microsurgical technology and techniques, indicates that the least of the patient's problems is surgery. Surgical morbidity and mortality is arbout 2 % (but up to 15% for the most difficult of aneurysms), yet, it is the most frightening part of the whole story. Surgery is, in point of fact, the least threatening treatment. Patients and their families always look at the surgeon strangely, when he says, "Surgery is the easy part overall! Don't worry so much about that part of it; much more difficult hurdles await during the next few weeks of therapy.

Preventing Hydrocephalus

20% of patients will develop (brain water) clogged CSF outflow passageways, causing what is called "communicating" hydrocephalus. Initial treatment might consist of the placement of a "lumbar drain" (i.e. an ongoing spinal tap for 5-14 days.) If the hydrocephalus persists, a CSF shunt (a tube, draining CSF from the ventricles in the brain, tunneled beneath the skin, into another location such as the chest or belly) could be placed for permanent drainage.

Preventing or Fighting Vasospasm

The real killer, after re-bleeding has been prevented, is vasospasm. Vasospasm can easily cause second strokes, when a brain blood vessel becomes so constricted that blood is prevented from entering the brain altogether.

When an aneurysm ruptures, blood escapes and does not go into the brain, but rather around it. The brain, like a permanent embryo, is filled with and surrounded by water. When blood gets trapped there, the red and white blood cells die and eventually disintegrate within 2 to 5 days. Imagine that each blood cell is nothing more than a simple bag containing a complex variety of chemicals, some of which are very toxic to the "smooth" muscle layer of the blood vessels arrayed around the brain. When these vessels are bathed by such a combination of toxins, their "smooth" muscles contract, causing the vessel to become so small that blood is prevented from traveling to the brain tissue beyond.

Despite more than thirty years of painstaking research into the problem, only three clinical treatments appear to be effective in concert: the so-called "Triple H" therapy.

  • * Hemodilution -- lowering blood viscosity (thickness, like thick soup, causes increased friction among blood cells, thereby reducing blood flow and threatening blood supply), reducing its minimum ("hematrocrit") to around 32. High blood viscosity is extremely dangerous in the face of potential vasospasm.
  • * Hyperdynamic State -- Increasing the amount of blood pumped to the brain by the heart ("cardiac output") appears to keep the "pipes" open by maintaining a continuous head of pressure in the system, and thereby preventing vasospasm. A cardiac output is around 5 liters/minute (JM: Is this the normal rate, or the rate desired to be achieved ?). With a combination of intravascular volume expansion (lots of I.V. fluids) and heart pumping medications (such as "dobutamine"), such a hyperdynamic state can be achieved.

Predicting Outcome

Let us assume that the patient has arrived at the hospital, been seen and evaluated by the doctors. They have news for you, and your decisions will depend on what they tell you. The doctor can predict with a fair degree of accuracy, what the future holds, even in the best of circumstances. He will count on at least two factors: (1) the Hunt-Hess score, and (2) the CT Scan grading scale. He will then factor in such things as added problems, such as hydrocephalus, general medical condition, age, and try to offer a realistic outlook for the patient.

Hunt-Hess Score - a way to measure to status of the patient after arriving at the hospital. In general, patients with scores of I - II do well, while others do not. The scores are:

  • Grade 0: Only the morbidity of surgery and that of natural history apply.
  • Grade I : After a small SAH, the only symptom is headache. The patient is neurologically normal.
  • Grade II: The patient is not all there. "Goofy" is a good way to describe this patient.
  • Grade III: Not only "goofy," but the patient has specific weakness of an arm, a leg or the face.
  • Grade IV: Severe neurologic deficit. This patient has great trouble.
  • Grade V: Vegetative state. A most horrible outcome.

When I was 40, in 1997, I had surgery for a very large aneurysm. It was on the carotid artery in the left section of my brain. It burst when they opened my skull. They put two clips in. The doctor told me it was congenital and that all of my other arteries looked fine. I am one of the lucky ones as my side effects are relatively minor. Other than the loss of some sense of smell, a need for more sleep and a little difficulty with short term memory I am fine. However, I have noticed a "dampening" of my emotions. I no longer really feel things, i.e. grief, anger, joy. I had no real problems immediately following surgery except for swelling of the brain that lasted quite a while. I wonder if the aneurysm and subsequent surgery destroyed or diminished my ability to feel things to any great extent?

I'm 40 y.o and underwent repair (clipping) of 2 x6mm aneurysms on my fright frontal lobe. Recovery has been good and the wound is healing well. 2 days ago (alomost 4 weeks post op. I woke to a puffy and swollen eye below the aneurysm site. Almost a bruised feeling and the sweeling is similar to the week after surgery.
My gp suggests it may be just a shift in fluid from the repair and has given me antibiotics.
Is this anything to be concerned about? I'm having a ct scan in 2 days and follow up with neurosurgeon later this week but the belated sweeling is causing me concern.

My 54 year sister had 3 ruptured aneurysms in the space of 6 days. the last one ruptured in the ER. This was 3 weeks ago, they just removed the tube and will probably put in a shunt. The doctors said she is a miracle. They have never seen a person survive 3. She has short term memory problems, that's it. What type of rehab will she go thru? Should I be screened?

My best friend had brain aneurysm surgery about 2 years ago and when the Dr. realized that he had slight bleeding, they decided to take out the actual aneurysm. Since the surgery, he was good for a few months after which they had to put him under the ICU and he has been under it since. What I want to know is, how long can a person stay under the ICU, would he ever survive? If he does come out of the ICU, would he be back to his normal self?

My friend was diagnosed in May with an optical aneurysm and has since been put on Plavix, told to drink two beers with her dinner, don't bend over, don't bump her head and a long list of other things. She has been told the Dr's are going to put shunts in her kidneys and a stent somewhere in the coarata artery? Basically she will have 4 surgeries all at the same time but a date has not been set yet. Does this sound normal that a Dr. would wait this long to fix something so serious?

My step-mother was perfectly fine and then was all of a sudden complaining and saying it felt like her head was going to explode. Then all of a sudden she wasn't making any sense and could not stand up any longer. Just got a call from my mother. My parents live in Illinois I am in california. My father has a prostate procedure done this morning. While recovering (outpatient) his head and face began swelling and he has slurred speech...They are now rushing him to a hospital (he was in an outpatient surgery center) saying he has bleeding around the brain. The doc is sure that it has nothign to do with the prostate procedure. My dad is 80, mid stage alzheimers and has had many mini strokes (TIA). What should I expect? Should I go now if I want to see him alive? He is alert although a little confused.

Just so people out there know that miracles do happen. My sister aged 54 had a grade v Sah 9 months ago. Its been the worst time of our lives but after all that has tranpired - blindness from Tersons, Hydrocephalus, Pneumonia etc she can now walk, talk, read, understand, spell, count & laugh.Even her short term memory is slowly improving. I'm in absolute awe of the recuperative powers of the brain.

I had 2 clips put in my left side of my head, say middle, left-side, about 3 years ago and since then I have intense, shooting pain at the exact or near exact position where the aneurysm was. I have been diagnosed with a 7mm Aneurysm ACoA. It is shaped like a fist and is inoperable to clip as the neck is wide and located at the arch of the arteries. The interventional radiologist is planning to place a stent and coil in the aneurysm to help save the artery. I am a 49 year old breast cancer survivor (3 years) and now this. Surgery is this Monday. I am really scared.

I have suffered from headaches for 30 years and back in September of 2009, my headaches started to change. One pill was not working anymore and the headaches were different, so I decided to walk into the emergency room one afternoon. I left work and did just that. They sent me for a scan and they saw "something" in my brain, so they proceeded to do an MRI which showed that I had 3 brain aneurysms. They told me that the headaches were not related to them. They put me in ICU for 24 hours and then I went home. I was sent to see Dr. Linfante who is the leader of the coiling procedure. After an angiogram, he decided that the locations and shape of my aneurysms were going to require brain surgery. Only 2 of them were big enough to require the surgery. Went to see a couple of surgeons and had a premonition, so I decided to go with Dr. Roberto Heros. It was the best decision of my life. Dr. Heros and his team did a terrific job at Jackson Memorial Hospital. I was in the hospital for 4 days and feeling good. I also have to thank my family for being there through the entire process. It is very important at a moment like this because it is a dangerous surgery. My daughters were incredible. Was driving a few weeks after. A month later I was back at work feeling great. The only issues that I have are expected after this surgery, which are a little short term memory loss which I help by writing everything down and also the side where my surgery was performed still is a little tender, but, other than that, I'm fine. I exercise, changed my eating habits, don't smoke anymore, drink a lot less and keep my high blood pressure under control. Aneurysm #3 was as small a a pimple, as my Dr. described it and he told me to have an MRI in about a year from now, to see if it's grown. I am 62 years old and going strong, thank God. I had Aneurysm surgery on January 23 and doing well. However, wanted to know, when can i go back to work. My doctor wrote that i can go back to work on June 1st. However, the insurance company is not processing my disability claim. So, what is the minimum and maximum period to take rest before joining in duties. My dad 55yrs old just got out of the hospital. He suffered from an aneurysm behind his right eye. They tried to coil the aneurysm first but this was unsuccessful (the aneurysm was very oddly shaped and would not hold the coil). They then did brain surgery to repair the aneurysm. The surgery was successful and they were able to clip the aneurysm with-out it affecting his eyesight. It’s only been a few weeks since his surgery and he has some short term memory loss. Had an aneurysm rupture March 2009. Was doing just fine until January 2010 had a dizzy spell and fainted,hit my head on kitchen counter corner that knocked me out unconscious for about 40 minutes. Woke up and felt my head and it had two cuts, blood on the floor, called 911, EMS to me to hospital, there they did CT scan said all was okay. Ever since the fall I have felt this floaty moving sensation around the craniotomy site. No one believes me, told doctors about it they didn't/don't seem concerned. I can feel it right now even though I feel okay otherwise. Didn't suffer any stroke or neurological insults from aneurysm, just easy to fatigue. This floaty feeling worries me.

Early the morning of March 3, 2010 it felt like someone hit me with a hammer on the back of my head. Assuming it was just a headache or I pulled something in my neck I went to the kitchen to take some asprin. After taking 2 asprin and a drink of water I laid down on the couch. Started feeling sick to my stomach, called for my son and then 911. Transported to hospital, then antoher and finally toJackson Memorial Hospital where they saved my life. They removed part of my skull and clipped the aneurysm. By some miracle I have all my memory and all my motor skills. Dr. Sultan and his staff were wonderful I see them in 6 months for an angiogram. I was truly blessed to have such great doctors with great surgical skills. After 30 days in the hospital back to work.

I'm a 49 year old female with no former history of aneurysm's. One Sunday I had a very dizzy spell, so dizzy it made me instantly sick. I went to the Er and was taken to the CAT scan and transported to the local hospital where they did and MRA to find I have an aneurysm in the carotid artery in the right side of my brain. Then an angiogram was performed and I found out I was alergic to the contrast dye they use for the test which is also the same they use to do the coiling procedure which they could not do on me, so the clipping procedure was elected. Now I'm going through therepy. Ever since the surgery I have had a funny since of smell, I keep smelling this same odor that I smelled in the hospital when the meals were served. A strong greasy type smell. Has anyone else noticed this? I hope it goes away. Now comes the rehab to get back on my feet. Never expected this at 49.

What to expect (4/15/2010)
My 26 year old son's 22 year old girlfriend has been told she has a growing aneurysm and needs surgery. She is waiting for the insurance process to go through so she can go to UCLA for the procedure. What I want to know is what my son should expect. I belive this is going to be a very serious surgery but how serious. How long is the surgery, how long will she be at UCLA and after that? What happens?

My mother passed away from a brain aneurysm about 9 months ago, but I still don't feel peace about it. The doctors never tried to do any surgery or try to save her life, but instead just let her lie there and kept telling us that she was dying and there was too much bleeding in her brain. But when I read stories of survived brain aneurysm victims, they all say that they had surgeries and that the doctors did everything in their power to bring them back. How bad does a brain aneurysm have to be to make doctors not try at all? Often times I feel like we could have done something more, but then doctors kept saying there was no hope at all. Is this possible?

I was 13 years-old when I had a brain aneurysm and a stroke. The AVM ruptured and affected my speech, learning, motor ability and memory. 6 years after it I still suffer from short-term memory loss and have lost some of my long-term memories as well. Also, I lost my prefreal vision in my right eye. They put a drain in to remove the blood on my brain, and did surgery to repair the damage. But they also did a Gama Knife Procedure to cauterise the vains so that this would not happen again. I was allergic to all the seizure meds they gave me,along with most of the other medication. The mood swings would come and go, it would all depend on my day. It was hard on my parents and my family, but they stuck through it and supported me. It's hard to suffer from this then try and fit back into a normal life. When your that young it tramatizes you! But through theropy and the support of family and friends, it's possible to get through it and live a relativly normal life. You just have to learn to live with the aftermath of this condition...

Is it a likelyhood of a second aneurysm with only one open carotid artery?

My wife had an AVM removal about a year ago. Now she's haveing a lot of bad seizures and mood swings all the time. She also has an anurism, and a kiro maulformation. The anurism is behind her right eye and the doc says it would be safer to leave it alone because it is concaved. What do you think about that and the kiro maulformation? Also she has been put on several meds for her siesures but they all make her very sleepy all the time and we have 3 kids that need her attention while I'm at work. Can you suggest some meds that are safe and non drowsey? Please help us I need my wife back and I can't stand to see her suffor. I would appreciate any suggestions you could give me. If this is in the family, is there a way of finding out if you are have this potential issue. Can they test for something like this. I have a cousin who has had mulitple brain bleeds and I also had an Aunt. This is on both sides of my family. What can I do or is there anything we can do?

It's behind my right eye and small and I can't make up my mind what to do about it....fix it with a 3hour procedure, going throgh the skull or watch for 6 months and see if it grows. I had a really bad headache that woke me up and continued on for about 12 hours. Not sinus, not migraine just lots of pain on the righr side of my head. Since then, I am still getting headaches but none like the first one. I'm afraid to do and afraid if I don't get it fixed. Any advice, Pat

my mother in law was diagnosed with a rupured brain haemorrage she had surgery to treat it. but during surgery the consultant found another couple of brain aneryurisms. one of them he is concerned about he tried to treat it also but her blood started to clot. he said he will repeat this procedure in 4weeks time. what happens if this rupures before she gets it repeated?

My husbandis 32,with family histroy ( father, 2 uncles) who died of ruptured aneurysm of the brain, he now found out he to has a brain aneurysm. He has head aches, stiff neck back pain, as well as some joint pain in the legs. Is this severe, should we be more worried than we already are.
