ARTERIOVENOUS MALFORMATIONS (AVM)

Arteriovenous malformations are masses of abnormal blood vessels which grow in the brain. They consist of a blood vessel "nidus" (nest) through which arteries connect directly to veins, instead of through the elaborate collection of very small vessels called capillaries. Some people are born with the nidus, but as the years go by, it tends to enlarge as the great pressure of the arterial vessels can not be handled by the veins that drain out of it. This causes a large collection of worm-like vessels to develop (malform) into a mass capable of bleeding at some future time. These malformations are most likely to bleed between the ages of 10 - 55; after 55, the chances of bleeding diminishes rapidly. Before 55, the likelihood of hemorrhaging is betweeen 3 and 4% per year (with a death incidence of about 1%). Once an AVM patient has hemorrhaged, the risk of having another one might approach 20% during the first year, and gradually lessen to about 3 - 4% over the next few years.

AVM s can occur in any area of the brain, and may be either small or large. When they hemorrhage, they usually do so with a limited amount of blood, unlike the hypertensive hemorrhages of other stroke patients. Loss of neurologic function depends on both the location of the AVM and the amount of bleeding. Many patients have very small hemorrhages, often multiple. They may display convulsions before even knowing about the presence of an AVM. Some patients suffer with headaches, often unrelated to the AVM which are usually found with a CT scan or brain MRI. In rare instances, children are born with large AVM's and are found to have heart failure because the malformation makes the heart work beyond its capacity.

These lesions are surrounded by a very discrete layer of abnormal, nonfunctioning brain tissue, thus allowing their removal with relative safety to the surrounding brain. This factor is of the utmost importance to the brain surgeon, who can take advantage of this natural separation between normal brain tissue and the abnormal vascular malformation.

TREATMENT OPTIONS

Radiation

If there is a very small AVM, and it is deep seated in the brain, the patient is fortunate. It is possible to give focused beam radiation to the malformation, and avoid surgery. Within two years the malformation will most likely disappear.

Embolization

Larger malformations may be made more surgically manageable with a technique called embolization. With this procedure an angiogram becomes a therapeutic tool. The interventional angiographer is capable of filling the malformation with agents which help decrease the blood supply to the malformation (coils, glues, plastic spheres, balloons, etc). This makes surgery easier in some cases. The technique has been used as the primary treatment as well, and has apparently been successful in some cases.

Surgery

Perhaps surgery is still the best way to go if the decision has been made to do something to eliminate the AVM for good. Surgery cures these lesions by totally removing them, thus disallowing them from ever recurring again. The author's personal bias (quite strongly held) is that most AVM's are best cared for with surgical removal. Even with patients who have large and complex AVM's, surgery provides the cure when the malformation is completely removed.

Today, with the assistance of BrainLab's most sophisticated equipment and computerized techniques, an AVM removal has been greatly facilitated. Here are two phases of an AVM removal:

Leave It Alone

Some lesions, because of their size, location or behavior are best left alone. In these cases, your surgeon's judgment is of the utmost import.

In other cases, courses of treatment may be determined by the age or medical condition of the patient. An example might be that of an AVM being found in a 70-year old woman suffering headaches after her heart surgery, having had no prior history of problems relating to the malformation. Another example might be the case of the 52-year old businessman with a very large and complex AVM located deep in the brain, involving the vessels that drain to the brainstem and/or speech areas of the brain.

my son three years ago, while he was 33 years old, burst an AVM (in the brain). Phisically he is fully recovered, however suffering from mainly short time memory problems. He is taking different pill aimed to assist Elzheimer problems, however no major change could be noticed.
I am looking for assistance in identifying further possible means that could help him.

I hope I can be a little bit of bright light for those of you suffering with an AVM. I suffered an AVM hemorrhage when I was nine years old. That was in 1990, when medical technology was not that great. The artery burst, leaving me without the use of my left hand and with other minor problems, such as a left visual field cut. Don't let anyone tell you that you cannot make something of your life. I also have epilepsy, which is now controlled. I was in a coma during the surgery, so I have no recollection of its removal. To make a long story short, I recovered to the best of my ability. I put myself through college. I got married three years ago, and now have a one year-old son. The pregnancy was not easy, but I did very well. I had a perfectly natural labor, no drugs or anything.
My advice to you is to go to a neurologist if you suspect you may have an AVM. I had absolutely no idea I had one before it ruptured. If we had known, it may have minimized the damage, but I'm grateful things turned out like they did. I had lots of therapy, but I did regain a lot. After AVM rupture (7/23/2010)
My 8yr old son had an AVM rupture this past October. He had surgery to remove it and it appeared he had no negative effects afterwards.It is Now July and he's starting to stutter a little bit and repeat his words. Can he develop speech problems now? The doctor said he probably wouldn't be able to speak when woke up from his coma or surgery...but he did well after all this.

Hello, My daughter had a AVM burst at the age of 28while she was 36 wks pregnant with my now beautiful 17 month old granddaughter , we learned she is now 2 months pregnant now , I am praying there is no threat to her now with any future re occurances of the AVM please if nyone has info please comment . thank you Mother

AGE 33 I WAS VERY SICK LOTS OF SPINAL ND NECK PROBLEMS DECIDED TO GO TO EMERGENCY ROOM GOT THERE ND STOOD IN NORTH SHORE HOSPITAL FOR AT LEAST 6 MONTHS AFTER 2 EMBOLIZATION ND RADIATION THE AVM WAS REMOVED I WAS PARILIZED FROM MY LEFT SIDE ....AFTER FEW THERAPHY GAIN BACK MY MOVEMENT BUT NOT FULLY OTHER THEN THAT IM OKAY.......3 WEEKS AGO AFTER BEEN SO WELL SUFFER TWO SEZURE SO IM A LITTLE SCARE AND WORRIE

My little brother (19 years old) has been diagnosed with AVM at the age of 13. He had over 10 embolisations since then and unfortunately 2 really bad stokes within the last 2 years. However the doctors didn't think he will survive he is fighting really hard and getting better day by day. The last diagnosis made by his surgeon said that he would need an other embolization in order to solve the AVM. This operation would be extremely dangerous (so were the others) but we would like to get a second opinion from some other specialist. Hi, I was just browsing around to see if I can still buy some kind of insurance with my episode and saw your questions. I am not a doctor but can share my experience. I found out I had AVM in my right brain at the age of 23 in Japan when I was studying abroad. No prior symptoms at all. I passed out at home one day because it bled and was found 3 days later, thanks to my friends. Just to make the story short, my surgery just so happened to be in one of the best Neurological Hospitals in Tokyo, Japan. The size of my AVM is a little bigger than a quarter, smaller than the dollar coin. It took around 8 hours even though they said 3. I was in the hospital for about a month (ICU, then recovery room). I believe it was a success because I am here to tell you my experience. After the surgery, I had problems coordinating because it's located beneath my peripheral, but it self adjusted as the years passed by and I am able to drive and work like a normal person. If I didn't tell anyone, no one would suspect I had the AVM at all. The down side is I have occasional migraines which I never had before, but it also subsided as well as Epilepsy, which I am still under medication for. As long as I take my meds, I don't have any problems. I had a tiny "residual" which have less than 1% chance of bleeding again and it was removed by using the "Gamma Knife"in the states.I guess I am very lucky to be able to function normally after such a big brain surgery. So with the right treatments, I am sure your children can too. I usually don't come on this forum at all, but if you need more info, I'd be glad to tell you my experience. Again, I am not a doctor, so I can't treat you or suggest anything, all I can do is tell you what happened to me that might help you understand it more. I want to know if there is something that can be done to fix it? The doctors are not being straight forward about it and are not telling me anything that I want to know! At 10 days old it ruptured and he had to be hospitalized. I really need some kind of information. When I was 11 years old during an CT Scan and an MRI performed to follow up on Miagrain headaches the Dr found an AVM in my brain. It was located in the area of sight. After visiting a pediatric Neuro Surgeon in "Chapel Hill" and him reveiwing my case alond with other Neuro Sergeons over the world it was decided that it would be best to leave the AVM alone due to the location. I am just wondering how often should I have an MRI to check up on the AVM.

My 14 yr. old son has an AVM. on the left side of his brain. We found out the end of april when it started to bleed they releaved the pressure put him in self induesed coma for few days. he was in the hospital for two and ahalf weeks then released him. they want to do the surgery but are waiting until he is stronger so his recovery will be greater. I'm scared for him. does anyone know what will i need to prepare myself for? The doctors say seizures, short term memory loss, sence of smell or taste ect. (is that all?)

My daughter had her avm removed 3 years ago and had a 3 year folow up angiogram and the day following she experienced a seizure. The doctors are telling her that the seizures can now start to happen. Is this true??

My husband suffered an AVM 6/8/2008. They did embolization(glued)the area. I thank God he is alive, but the mood swings are not to be believed. He is now 63. We no longer have sex, he doesn't even put his arm around me. He doesn't smile, laugh, and joke around. He is still in therapy for this. The problem is I need help.

My son (21) suffered intracerebral haemorraghe during sports; it had occurred because of AVM. The AVM was on the left side of the brain. He had afasia. He had surgery to remove both the AVM and the haematoma. He recovered nicely; his speech came back and he has no neurological problems. We thank God for this outcome....I am however still concerned about recurrence of AVM. Does anyone know something about this phenomen? The neurosurgeon is very vague about this subject and scheduled another arteriography.

Recent AVM finding (4/28/2010)
On 4/22/2010, I had an MRI because of some concerns that I might have some type of Auto Immune Disorder. I just found out yesterday that I have a subcortial AVM. My doctor never called me, I found out what it was by research. Now I have to go to a neurologist. Im really scared, I have a three year old son and I would hate to be driving and have a bleed out or a seizure. Any suggestions?

Recurrance? (4/7/2010)
I have had 2 AVMs. One bled when I was 20, surgery fixed it...so I thought. 2 children and 12 years later I noticed my headaches worsening, I knew something wasn't right. Angio confrimed an AVM. I had surgery #2 in 2007. I have been told 2 different things about it-either it was residual from the first sugery in 1995 or it recurred. Now I have a stabbing pain in the same area as my AVMs and am scared of a recurrance...anyone out there know anything about them????????????

My 13 year old daughter was told she has a AVM on her left distal radius(wrist). We can not find a doctor in this field to help her. Everyone who looks at it is in all at the size and condition of the AVM. We are told it is one of the largest anyone has seen. So, Our problem is does anybody know of any doctors in the NJ area????

I had carried my 2 cm (less than one inch) AVM for some 56 years. I never had a stroke or seizure. I just had it removed this afternoon. Total radiation time was about 30 minutes, divided into 6 veseels that needed to be closed or shrunk. The preparation time such as an MRI with contrast of couse added to the total time. Immediately after, I was able to drive home, although my wife disallowed me. I feel fine, as if I was back to my teenage years. No numbness, no seizures.

Hi my name is Tracie and in July 2006 whilst at work I had a stroke caused by an AVM in the brain, I was aged 41 at the time. I received sterio tactic radio surgery in February 2007 and in April 2009 was told that the surgery had worked and the AVM had gone! I've been left with what I consider minor side effects, ie loss of periferal vision to the right hand side, extreme fatigue at times, short term memory loss and epilepsy, to name a few things. But considering what could have been I consider myself very lucky to still be here. Anyone contemplating whether to have the sterio tactic radio surgery I would say go for it, in my case I had no other option and with a family to think of there was no way I was going to not accept any treatment offered. I now lead what I consider to be a normal life, bad days now and then but I now realise how precious and short life can be. There is definately life after a stroke.

AVM...what to do (3/25/2010)
In Dec 2009 my son had routine surgery on his wrist/thumb. The day after his release he had a GM seizure. A MRI/Ct scan and a cerebral anagram reveled he has an AVM deep within his brain. We have been to Drs and surgeons and specialists in Phx and SLC. Dr. Spitzer(sp) recommends surgery right away-giving little hope for a good outcome..paralyzed-speech problems etc.etc. My son wants to wait and prepare himself physically and spiritually..."get healthier" take anti-seizure meds and live life a bit more .He seems to be having a few personality changes and is sometimes very erratic. He is in his early 20's and needs some help in making this life altering decision. Do we get healthy and wait for technology to catch up to AVM's or should we risk the surgery...HELP

Hey my name is kristy from columbia,sc an in 2005 I had surgery to remove a large AVM. Like most people who are told you will die unless you have surgery ,I was rightfuly scard so I waited til the last moment that almost cost me my life and my kids a mother.The day of surgery it burst and I lost alot of blood ,causen me to have a stoke and from what I was told I died and was brought back.My live as i know it was changed forever.Now paralized on my left side .I m haven trouble finding a job and help that Need to take care of my two kids.So to any one that reads this your body tells you when some things not right,so listen and dont wait till to morrow to fix it.It could be to late.Take care and god bless.

My fiance has had 4 intracranial strokes in the past two months. The last one was severe and he has not recovered too well. What can I expect as for as his recovery.
